Save Non Editable Copy of the Form

I have a PDF form with empty fields.

After the form is filled in I want people to press save and have the form generate a new copy of the document for saving and the original template to remain unchanged.

I would prefer the new copy to have static elements, ie. non editable.

I hope this is clear.
