I’ve tried looking at the 360 Degree Feedback form but unfortunately, I did not see this as an option.
Let's look at the example:

Cleanliness
How can I create the survey design question above? Also, does Adobe FormCentral provide Skip Logics, Filters etc?

We just added the ability to do page skip logic in our release last week. It should help you meet the requirements that you described earlier. I wrote up an explanation of the feature here: http://forums.adobe.com/thread/869959
Likert should be arriving at the end of July if all goes well.

We just added support for rating scales (aka likert). I wrote up a forum post on it here: http://forums.adobe.com/thread/884116
Also check out the new "Summary Report" tab - we now automatically generate charts for each question that show how many users picked a particular choice in a question.
With the additional of these and our previously announced page skip logic feature we now have a very compelling survey solution.
