What's the right way to use InDesign to create a Kindle book?

Hello everyone!

I've created an epub for iBooks which I am very happy with.

I now want to create a Kindle version of my ebook. I'm using InDesign CS5.5 on a PC and have installed the Indesign Kindle plugin v0.94. As a quick test I took my Indesign file and exported to Kindle with messy results. The effect is that the chapters are not separated, instead I have one long continuous page.  Some of the images appear in the wrong place, instead of appearing between two text paragraphs they appear at the end of the chapter. Also the inbuilt Table of Contents  is greyed out /doesn't work despite applying me having applied the correct ToC style to the chapter headings (it worked in the iBooks version.)

I realise that I have to make changes for the Kindle version.

Q1) How do I start each chapter on a new page?

Q2) How do I make images appear exactly where I place them in the middle of the text (and not at the end of the chapter)?

Q3) What is the maximum pixel size for images I should use, 800x600?

Q4) How do I make the inbuilt Table of Contents work?

Any help appreciated.

Why not try converting the epub to mobi with something like Calibre?

The plugin is not worth using. I make an epub export and then use the KindleGen software to make the .mobi file.
