What format must the html be in for the link to my form work?

Does the form created in my account named 80th Celebration work in a webpage created with the criteria below?

I cannot get it to work in my website. Not by just the link or embedding the script. Have the form shows up with script and the link will not work goes to a your page saying this form does not exist.

    Hi,

     

    You should be able to embed your form by copying the embed code from the embed dialog, which you get to by going to the distribute tab, and clicking embed.  Note however that the embed will not work on an HTML file opened from your local drive.  It must be opened from a web server.

     

    If you tried that, and it's not working, can you email me (trein@adobe.com) the URL of your webpage?
