DW -  Date Picker Calendar help

I have got the Date Picker Calendar from the widgets site (originates from the Jquery site) working to input a field in a form, I have it set up for 2 fields but can only get it to work in one instance?

 

The 2 fields I have to fill are -

 

Start date

End date

 

But the calendar will only pop up on the first instance the code is entered on? looking at the code it is a function being called so why can it only be used once?

 

Can anyone help me?
