you have many possibilities, for example you ask your hosting provider, which offers forms processing script and instructions, I'm sure. On the other hand you can try Adobe Spry here http://labs.adobe.com/technologies/spry/demos/formsvalidation/ or "search engines are your friends" by using these (or ather keywords) "dreamweaver contact form template" and you will get e.g. http://www.dreamweaver-templates.net/contact.shtml.

honestly, I do not think it's such a good idea to "graft" a submit button onto an image. I would recommend that you better use for this part on of the solutions above.

only to complete: When you are using images, there is a possibility to use the "hot spot" technique too. But unfortunately, you can only link to another file or another page.
