Acrobat 8 Pro, Office 2003, Windows XP. The Adobe toolbar etc is not available in Outlook - nothing Adobe is visible anywhere (not under disabled items, COM add-ins, etc). The toolbar is ok in the other Office apps. I went through this article - http://helpx.adobe.com/acrobat/kb/no-pdfmaker-office-2003-and.html ("PDFMaker Unavailable/Office 2003 and earlier...."), did most of the applicable suggestions; no luck.
In the registry there exists this key: HKLM\Software\Microsoft\Office\Outlook\Addins\PDFMOutlook.PDFMOutlook . However there is no file named PDFMOutlook.dll on the PC. The other Office apps have "PDFMaker.Office.Addin" keys, and the file PDFMOfficeAddin.dll exists in C:\Program Files\Adobe\Acrobat 8.0\PDFMaker\Office along with dll's for each of those other Office components (e.g., PDFMExcel.dll, etc). But no Outlook file.
After earlier repairing Acrobat to no avail, I tried deleting the Outlook reg key above then running the Acrobat setup in modify mode - I uninstalled just that Outlook component, then reinstalled it. It put the reg key back, but there is still no PDFMOutlook.dll file on the PC. I also looked in the above path of other user PC's here that run Acrobat 8 Pro & Standard, and noticed they do not have an "outlook" file either (which is why I didn't bother to uninstall the entire product - it's a pain to get the setup disks from our helpdesk.) I wanted to take a look at the PDF package feature for email messages as something possible for our users to use, so simply printing to PDF is not going to help.
Any suggestions? If there should be a PDFMOutlook.dll, is there somewhere I can download it from?
Can you please check if you have PDFMOfficeAddin.dll located in C:\Program Files\Adobe\Acrobat 8.0\PDFMaker\Office?
If there is then open your outlook.
click on help and select about outlook.
There should be an option about disabled items.
If acrobat is there check the box and select enable.
If no acrobat present select the option to add
follow the path on top and select PDFMOfficeAddin.dll and enable it.

Thank you for your response. Yes, as indicated, PDFMOfficeAddin.dll is in C:\Program Files\Adobe\Acrobat 8.0\PDFMaker\Office, but there is nothing under disabled items.
I attempted to add the above add-in manually as you indicated. It's Outlook 2003, so I went to Tools>Options>Other>Advanced Options>COM Add-Ins. I clicked Add and located and selected PDFMOfficeAddin.dll. After clicking OK it gave no error, but the add-in was not added to the list. Tried same after de-selecting all add-ins under COM Addins and the Add-in Manager, but no luck (restarted Outlook between attempts).
