Generating repetitive blank forms

I am trying to turn our internal audit report into an e-form.  Generating the forms & fields themselves is no problem, however the Findings Report (the last page of teh hard copy) needs to be repeated, once for each individual audit finding.

 

I basically want to create the blank, fillable form and have a button/tool at the bottome that - when the user presses/selcts it - generates another copy of the blank fillable form & inserts it after the previous one.

 

Not sure if I'm shooting for the stars here, but any assistance is gratefully accepted!

    Only possible if the users have Acrobat, or if the form was created in

    LiveCycle Designer.

    If you have Acrabat and the users will be using Acrobat, you can use the template objec
