opt/click doesn't always bring up the name dialog box when clicking on new layer mask. This is an on and off response.

Hello, please do not cry wolf and say bugs and buggy... your system seems to have problems that needs to be resolved first. You seem to encounter many issues that others don't. Let's get everything resolved, then you could start to hunt bugs again.

You would be better served working on one of these issues first to solve since it is likely causing several of your issues. There must be something different about your system causing the bottons to change style, the ui color, and other "buggy" behavior. I will again go back to the refine edge link to respond in one place for you.
