InCopy layout view different from InDesign

We are using the assignment workflow with InDesign and InCopy CS5 all on Macs.  When our designers create the layout and create the assignment file everything looks fine on their end.  When the editor opens the assignment file in InCopy the layout will be off either by just a few lines or massively off.  The fonts are all correct for both users.  There isn't any odd text wrapping added.  If the editor opens the InDesign file in InCopy it will appear all right.  This seemed to fix the problem once, but it's not working this 2nd time.  Is this a known bug?  Or is there a workaround for this?  Below are examples:

    Were these documents ever CS4 or earlier? What happens if the editor opens the INDD file in InCopy instead of the assignment?

    These documents have only ever been in CS5.  If the editor opens the INDD file in InCopy it appears correct like the 2nd example image.  I tried having her check out and check back in content while it was open and then close and open the icma file and the layout still looks incorrect.  This has worked previously, but did not work this time.

    Try exporting the INDD document to IDML and then reopening it.

     

     

     

    Try working on that.

    The IDML file shows the layout as incorrect and the INDD file still shows the layout as correct.

    Then there you go. There’s something wonky in the INDD file.

     

     

     

    Save the IDML file as INDD and work off that.

    This is a good solution, but we don't know how the INDD file gets messed up.  This has started happening with almost every project this designer works on.  Is there any setting that needs to be turned on or something the designer might be doing to create this problem?
