Cannot fill in a form

The purple bar on the top of the screen says: Please fill out the following form. You cannot save data typed into this form.

It doens't allow me to type in any of the needed fields.

    It's actually partially filled out with personal information. It was sent as an attachment for verification and to fill in some outstanding feilds. Above the top of the form it has a purple box that tells me to fill it out, that I cannot save it and that I will need to print it if I want a copy. But I have been unable to fil in any of the needed fields.
