I have a standard training simulation generated slide with a click box that has a success and failure caption. Things work as normal; both success or failure show when appropriate.
But I want to add a 2nd click box or button to play (show) an audio clip when clicked. However, when I add either a 2nd click box (no captions, no pausing) or a button, the above-mentioned failure caption will no longer show. The success caption will, however.

When you add a click box, whole area of the slide (except where click box is present) becomes "Failure Caption"
Just want to double check, Are you adding that 2nd click box or a button on the same slide ? (Slide on which you have the 1st Click box)
If yes, then when you have 2 click box on 1 slide, 1st click box's Failure Caption will not show because 2nd click box comes before 1st in Timeline (As you added 2nd click box after 1st)
So either you can use "Appear After" (Under Timings in Properties Inspector) of Click Box and show you 2nd click after 1st click box or you can use Rollover Caption to show you Audio.

In my experience, the layer order of click boxes can determine whether a particular Failure Caption displays.
If both click boxes must exist at the same time, try moving one above the other on the Timeline and see if your Failure caption starts displaying. Unfortunately, I've never been able to remember which one needs to be on top - I just swap the order until it works.
I've had mixed results changing layer order via the Timeline in the past, so worst case you can also try doing a "Send to Front" or "Send to Back" directly on the click box to force Captivate to change the layer order.

Changing the layer order putting the 1st click box on top did the trick. Interesting, I figured that as long as the boxes we're layered over each other, there wouldn't be any conflict. So the 'behind the scenes' workings of Cp prioritizes based on where things are in the linear timeline AND in the layer of the timeline? Again, I could see that as a problem if the clicks were overlapping on the slide, but never figured my scenario would cause problems.
Unfortunately changing the order of the Click boxes hasn't worked for me, I have Advanced Action on mine that once clicked the 1st click box hides and the 2nd click box appears in the same place as the 1st one was, so that I can get dual control on the section of the slide.
Can you tell me if this is what might be causing the failure caption not to appear, Can I also just add that the Success caption shows no problem.
Then probably something is wrong with the advanced action. I checked this out on 6.0.1 and it works fine. Changing the order of the click box will not help at all. You only have to be sure that the play head remains in the active part of the click box and that only one is visible at the same time.
