submitting forms for editing

I am new to boards but giving this a try. I am a physical therapist. I want to make a hand written form into an electronic form. then I want to send that form to an agency where they will review the medical content possibly editing it then returning it to me for review. If I approve when I come to the office they will print it and I will sign it. I would like to do this with PDF reader at the agency's end. I would like to use the form on an ipad PDF reader to fill the data into the form. I don't understand the lmitation I have read about 500 unique users or 500 edits. Can anyone explain this to me. I'm very confused about this. I will be using the form by populating it with data from each patient.

    I am assuming you have tested the form on the iPad and that it works. It was not clear since you keep mentioning the iPad.

     

    I am not a lawyer, but generally I interpret the 500 use limit as to when the form is submitted. You are using the form for personal use and not submitting to a system. There is another interpretation where the use is limited to 500 users. I have not looked at the license lately and it would be good to read it again carefully. I think you are fine, but it does not hurt to be concerned. If you have legal counsel in your organization, it might be worth passing it by them.
