Unsaved Metadata Still Unsaved

I'm running LR4 under MS Windows 7.  I just used the Metadata filter to select all images with "has been changed" as the status.  I selected all these images (about 100) and selected Metadata/Save Metadata to Files.  When the process was finished I had 7 images still showing under the filter.  Five are from an HDR image set with three CR2+JPEG, and the other two are individual HDR merges, as TIFF files.  Another is a single CR2+JPEG image, and the last is a single CR2 image (by this time I had stopped capturing JPEG images along with the RAW images).

I select these images again, either all together or singly, and do the Metadata Save.  For an instant the Unsaved Metadata cell icon disappears, and then it comes back again.  I still have 7 images showing the data needs to be saved.  If I look in my photos folders, the appropriate .XMPs show that they were in fact updated.

Can anyone tell me reasons for the image in the grid view still showing that the metadata needs to be saved?  I'm not getting any error messages or any other indication that there's a problem.  Thanks.

UPDATE:  it occurred to me that I should make a new change to the metadata of these 7 images, and try the save again.  I add a new keyword to each image and ran Save Metadata to File.  The 7 images disappeared from the Metadata Status filter window, the Has Been Changed count dropped from 7 to zero, and a message indicating that no images met the criteria appeared.  Success?  Nope, because after a few seconds all 7 images popped back up again, and the Has Been Changed count went back to 7.  This is bizarre.  Help!

I did a search for "metadata status" because I'm encountering exactly the same problem.  I don't know if LR has ALWAYS had problems saving metadata, but in LR4 I'm encounting situations where using the metadata status to select and update the metadata for images that have changed does NOT update the metadata for all the images.

Like the OP, I select a half dozen images, tell it to save the metadata, they clear, the filter shows "0", then a couple seconds later one or more of the images comes back.

Much as I think it was an interesting experiment for the OP to make additional changes to the metadata, that IS NOT REASONABLE.

I'm using .dng files in LR, so it's not having to contend with Nikon NEF files.

Does anyone know what's going on, and what's being done to FIX this problem?  'Cause this one seems a lot more significant than the trivial annoyances I've been asking about...

My guess: there was an error saving the metadata, which went unreported.

My hope: obviously, fixing the source of the problem, if possible, but also: I'd like to see Lightroom notify the user when such errors occur, preferrably with an informative error message.

So, whether it's not actually saving the metadata, or it IS saving correctly but reporting incorrectly, is this a bug that needs to be reported somewhere?  And, if so, can you point me to where I should report it?

It only affects SOME of the images that are being updated, and it seems that an image that doesn't report a correct update continues to report that it hasn't updated...

I tested last night, making a metadata change, updating the image on disk, and opened the image in Bridge.  The update was reflected...  I went back and forth several times, and the update APPEARS to actually happen, but for whatever reason, LR continues to report that it hasn't actually updated the metadata.

In this case, my guess was wrong. report here:
