It's not available as a separate plug-in. It comes built-in to Photoshop CS5. Your best bet, if you've managed to delete it somehow is to uninstall and reinstall.

Keep in mind that in Photoshop CS5 and ealier it can only work with 8 bits/channel RGB images.

Edit: Is there some possibility this was not ported to Mac for 64 bit operation?

you should probably update to 12.0.4

Photoshop, Help> Updates...

and 10.7 to 10.7.4

Apple Logo> Software Update..

my screenshots were taken with an sRGB 8-bit .jpg open

both Photoshops are 'vanilla' default installs, nothing removed

You need to run cs5 in the 32 bit mode to get the lighting effects filter as per the link Noel provided.
