I'm having a problem linking a TOC to pages in the document. I can do a few of them but then I select the text but I'm unable to select "new hyperlink". It is not highlighted to select it. What am I doing wrong?

Here is the printscreen of the Indesign file. I am trying to hyperlink the pages listed in the TOC to each respective page. (The TOC is in the green box)
I can use my curser to select a line and create a hyperlink. But this only works for some of the text, other text when highlighted does not allow me to select the the "Create New Hyperlink" button.
You cannot create a hyperlink inside an existing link; and the standard TOC settings make a link of every title. You can see so for yourself when you click inside a toc text -- in thr Hyperlink panel, the "create link" button is disabled, but the arrows left (select current link) and right (go to destination) *are* enabled.
you haven't answered the Alec question right.
If you create the TOC , from Layout-->Table of content , the each entry in the TOC is automatically linked however this functionality will not work in InDesign itself , you have to export the file to PDF to test it with "Create PDF bookmark" option checked in the TOC window.
So , Have you typed the text and created the Hyperlink manually or have you used layout-->Table of Content option ?
Secondly , how you are actually checking the TOC hyperlink are working or not ?
This is a file I inherited from someone else. It just appears to be a text block with a paragraph style attached to it. I did not create the TOC from Layout-->TOC. There are no links in the interactive PDF. Except for the few lines that I am able to attach hyperlinks to.
The first image shows the line of text highlighted and the hyperlink window open. I am able to link this to it intended page.
the second image shows another line of text hightlighted and the hyperlink window is open. The difference being I cannot select the "create new link" button to link this line to it's page.
