Trouble with form

I'm trying to embedd a simple contact form, made in html.

 

I've copied the html code inside the page in muse, and in the preview, or when I see it locally, it works just fine.

 

But when I upload to my server, the contact form keed appearing on the top of the screen, below my header. I'ts making me craky.

What am I doing wrong?

    Hi,

     

    Group the embedded html with the content on the top of it from Object -> Group option and let me know how that goes.

    There also might be trouble with the HTML you pasted in, there are closing "</body>" and "</html>" underneath the table and form code. Please remove these two tags and give it a try.

    I've tried both things. Still looks the same.

     

    In fact there's something even more strange. You, see, in my file I've put a box bellow the form (see the pic)Screen Shot 2012-08-03 at 5.40.03 PM.png

     

    When I see it locally, it's fine, again. But as I upload it, it goes away.

     

    I've cleaned my cache, deleted the file from the server and upload it again... and nothing...

    Hi,

     

    Export the whole web site again to a different location, desktop most probably, and if possible use an updated ftp upload software like filezilla and let me know of your findings.

    Abhishek, thank you very much...

     

    The form works fine now. I'm thinking that the problem maybe on some CSS rather than in the html itself... when I re-exported the whole site, I' fixed the problem.
