Audio and Video don't playback correct!

Yea so I import a .mp4 video and when I got to play it, the audio is always 1 second ahead of the video.

But when I play the source video the audio matches up correctly

 

Please help me I am using CS6 and the newest update!

    Due to lack of information, I can only guess, but these are the most likely causes:

     

    1. OE, operator error.
    2. Wrong source material with the wrong codec.
    3. Wrong platform.
    4. Wrong hardware.
    5. Wrong drivers.
    6. Wrong installation.
    7. Warped system.
    8. Software not up-to-date.
    9. Myriad other things, that can all be grouped under OE.

    and furnish the info requested in it, perhaps someone can see where the problem lies.
