Never had a problem wwith this issue before. Any help?
It's good that you write tight, but a few more details would help the rest of us....
Was this TOC working before, or is it new? Was the book and files created in 9, or rolled forward from an older version?
What's your environment: OS, FM patch level, networked files, etc.
Without more info, I'd suggest:
Assuming the TOC in exact the same book worked before:
* The Update Book dialog has a checkbox whether generated file should be updated
* In addition to that there are two lists in the dialog which allow you to exclude certain generated files from updating.
If someone else possibly did something to your book: Is the TOC really a TOC from FrameMaker’s POV or is it only named like a TOC? Generatable files have a different document icon in the book and you can right-click a "real" TOC and find a menu command which allows you to change the TOC settings.

I'm still having a problem. Here's some more information:
Files and TOC created in 9.
It is a "real" TOC, with the appropriate generated file icon in the book.
Operating environment is Windows XP.
I've set numbering to continue from previous page, small Roman numerals. Pagination to make page count even.
Ad Jeff mentions, you need to update. All the updates need to be applied in sequence, individually. Either through the FM interface Help > Check for Updates or downloaded here:
So it's a real generated ToC? Not just a regular .fm file rigged up with cross-refs and called a ToC? If so, give us a screenshot (using the forum web interface) when you go to update the book - your ToC file should be listed to be updated.
