I have two audio devices in my machine. The internal onboard Realtek (optical out -> surround setup) + Line6 UX1, external usb (for guitar inputs/amp sim).
I switch between them as needed. The flash player does this too, but always when I don't need it to.
Example. A youtube video finishes. The sound came from the speakers, as it should. I click the next one. It starts playing but there's no sound - except from the headphones connected to the UX1. This happens once or twice a day.
Does anyone know what's going on?

I just right click the speaker icon->playback devices->select device, make default.
When this happens the the correct device is btw still with the green check icon in the Playback Devices->Sound dialog. How I used to fix this was by opening that dialog, make the other device active (the one I don't want, the one flash decided to play through) and then change it back to the correct one again. Then the sound kicks in. I found a better way to fix this today though. One or two browser refreshes does the trick. Usually just one.
I also have a third device that shows up in the Sound dialog, the realtek analog out, but it never switches to that. I also never use that one.
I don't know what more we can do other than to enter a bug at bugbase.adobe.com and have someone take a look. I suspect switching interfaces while Flash is in use will be unsupported. In the bug report, it'll be good to note if this is new behavior (ie. it started behaiving like this in 11.3, but 11.2 and older were fine). However, the key to getting this fixed will be to get other users running into the same problem to vote for the bug.
