Need Help with Form Data-Please!!

I have setup dreamweaver with a localhost server and I am connect to an access database through dns. 

 

I created inserted a data object through the data object insert wizard.  When I enter data in the form and click submit, the data does not appear in the tables that I am connected to.

 

Can anyone help me?
