Actually Claudia's course on Lynda.com is about creating forms with Acrobat X Pro, and she completed it before InDesign CS6 was available.

Anne-Marie Concepcion discussed creating forms in her New Features in InDesign CS6 video on Lynda.com.

But Sandee Cohen wrote a dynamite article in the current issue of InDesign Magazine about creating forms in InDesign CS6 which is very thorough. The magazine is not available for free.

Bob, Thank you for the link and the response. I think I need a written tutorial or chapter from a book on this area so that I can quickly use it for reference. I will try to stop by Barnes & Noble tomorrow morning on my way to an appointment, and see if they have a book I can purchase.

I just filled out the contact form to get this resolved. My cc has been entered twice and the form was resent twice on one of those times. I hope to get the issue I need before I have to leave for an appointment. I will be working on my client's form while I wait for my car to be repaired today.

Thanks very much! I recieved the offer for $29 but that code was also rejected. It appears there is a problem with the online form. I've already paid the full amount for the subscription because i needed the article about interactive forms before I leave today. Maybe an adjustment could be made on my cc?

Thanks very much jmvrankin! They called me today and I decided to keep the two year plan that I paid for. Since I've been using ID since it's inception, I will value future editions of the magazine more than the past editions. Amy was very nice and helpful. I appreicate your help also!

Also, I was able to download the PDF by Sandee Cohen on Interactive Forms. Whereas I learned much of it the hard way yestereday , there were a couple of very important points that Sandee brought out which gave me direction on what I needed to do to make sure my forms were built right. And I was able to get some real billable work in today as a result. So you all are on my good list. xoxo
