converted file to word - conversion changed forms / header & footer detail

when I converted file to word - at section where there are forms to fill in - conversion has header and footer (which I cannot "Close") and that being said - what it is choosing for header has continued to be a header on the rest of the pages - as well as the footer - AND it has messed up the print - moved the print of the rest of the pages.  why ?

    What content is identified as being Header or Footer content is usually dictated by tags in the document and the margins.  You may want to double-click the content that's in the Header or Footer in Word, then cut and paste it into the main page at the appropriate place.
