Cs5, Epson 3800, OSX

I had Lion, Epson 3800 and CS5 -  no problems -  suddenly printing only portion of image in center of paper  or top edge depending on "center image" box checked or not -   used to be when this happened it was a Page Set Up issue -   no longer HAVE page set up....

 

So dumped Epson driver and redownloaded -  same problem

Purchased and Loaded Mountain Lion and reloaded driver - same problem

 

Printed out of InDesign -   no problem

Returned to PhotoShop - same problem   

 

So this is a setting issue in Photoshop -  I know I must have inadvertently changed something - but WHAT?

I want my Page Set up back!!!!!!   

 

Printing on Fine Art Velvet  17 x 22    image is 13x19 on 17x22 canvas 

rear load for the fine art paper

 

 

I have been printing successfully with CS3, the 3800 and various Macs for quite a few years and other incarnations of all of the above for a LONG TIME .... and now I feel like a complete neophyte -    what am I doing wrong?   

 

Please help I am losing money, going crazy and  almost sat on the studio floor and cried....   two days of trying small changes and drastic ones ....
