Education fulfilment form

Where is the education fulfilment form?  I've wasted over half an hour looking for it and see no option but to spend another half an hour on the phone asking where it has been hidden.

 

I tried searching in the forum.  When i entered "education fulfilment form" I got one response "Conversion errors...From PDF to Doc or Text".

    I received an email after placing an order with instructions.  Had to go into the support portal.  You input your scanned docs as a support case (that is the form).

     

    Step 1

    Scan your academic identification and save the file as a .PDF, .JPG or .GIF file. These are the only accepted formats. All forms of verification must be clearly legible and at a minimum, true to size.

    Step 2

    Sign in to the Adobe Customer Support Portal using the same Adobe ID used to place the order, and follow directions to submit your copy of your academic identification online. You will be asked to enter your Adobe Store order number, which will be located on the store thank you page and your order acknowledgement e-mail message.
