Save form name using field data

Is there any simple means by which a save button, and the resulting name of a filled form could be linked to particular data in the filled form?

 

Example:

 

Fillable form UserInfo.pdf has fields "LastName" and "DateOfBirth".     The user fills the form (Smith; 1972-07-15) and clicks the 'save' button.  The form is automatically renamed "Smith_1972-07-15.pdf".

 

A web search reveals only a few rather complex means of accomplishing this and only where a common server is used for data submission. 

 

This would be a fantastically useful feature.
