How create a Form for my app

Hi all DPS forum members, I need to customize your client application with data such as name, phone, address, and that this information also serves to create a link with your contacts and telephone numbers of emergency physicians, in order that they within the aplicacacion can make a call to your doctor or emergency servocio as 911.

My question is, this is possible with DPS and how?

You can try implementing it using an HTML form in HTML article or web content.

Yes, make a HTML Article or an InDesign article with a box containing the html file as Web Content. If you want a very easy-to-use form creation tool, I can recommend http://wufoo.com/ or Adobe's FormsCentral http://formscentral.acrobat.com.

Just remember to show alternate content for people reading the magazine in an offline environment. Here is a couple of ways to do that with javascript: http://stackoverflow.com/questions/2384167/check-if-internet-connectio n-exists-with-javascript
