Button Question

I posted this question earlier, but I wanted to rephrase it.

 

I have a form that contains multiple rows and multiple fields in each row.  There are instances where certain fields might need to bu duplicated and I would like to create a button that when pressed would duplicate the entries in the row above it.

 

EXAMPLE:

 

[name] [employee id] [title] [date worked] [ot hours worked]

[name] [employee id] [title] [date worked] [ot hours worked]

[name] [employee id] [title] [date worked] [ot hours worked]

[name] [employee id] [title] [date worked] [ot hours worked]

 

That is how the rows appear.  I would like to create the button starting on ROW 2 and when pressed will fill in the fields [name], [employee id], [title] with the information that appeared in the row above it.  This way, if an employee worked more then one day for OT, I can push the button and duplicate his/her info.
