You are seeing a problem in iOS. The PDF engine that iOS has built in, which is used by Mail, Safari, and many other third party apps, including the iOS Printing system, does not fully and properly handle the PDF specification. The iOS PDF engine will not display annotations ot form fields, even though the information is in the PDF file, and can be seen in Adobe Reader and pretty much every other PDF application available other than the iOS built-in.
We have informed Apple of this issue, and what they need to do to fix it, but they have not provided us any insight into when or whether they will fix it.
We are working on a workaround for Printing via AirPrint that we hope to have in our next major update later this year, but this will not work for sharing via Mail and Dropbox, thoiugh, because it would render the form fields and comments unusable.
