I´ve downloaded a document (from e-mail) and want to save it in my Ipad (ideally saving to a folder).

On your iPad open the document in Mail. Click on the PDF icon. It will be previewed in Mail. Click the Share icon. Then choose Open With, and select Adobe Reader. It will open in Adobe Reader.
You can create folders in the Documents mode of Reader. Click on Documents on the left. Click the Edit button (upper right). Click the Create Folder button and name a folder. Then click Edit again to move a file into the folder. After clicking edit, select the PDF file (you'll see a red checkbox). Click the Move icon, select the folder you want to move it to.
