How to Post Web Form Legal Contracts

Does anyone know of any web form contracts that are available (or can be pasted into Dreamweaver)? I've been uable to find anything.

 

I have a contract here http://www.eventlightingonline.com/bookings.html but the problem is that it doesn't include the disclaimers and cancellation policies in the email, nor does it send a copy to my customers. So far, it has served its purpose to commit customers (only because they haven't realized it is useless), but I'd like to know the proper method for accomplishing this, to protect both myself and my customers.

    There was another thread about this recently: http://forums.adobe.com/message/4731765#4731765

     

    The suggestion there was to use a service like http://www.echosign.com/ rather than attempting to hand code via php with databases and all that.

    The problem with echosign is that the customer has to contact us first with an email address. I'd like customers to be able to fill out their contracts while on our site (and while still feeling impulsive). For now, is there at least a way for the <p> text to appear on my form emails? That way I can at least send them a copy of the email I receive with the cancellation policies and other disclaimers without having to keep pasting them in all the time. That would be good enough for now.
