Thanks very much for your reply Altruisteic... much appreciated! I have done exactly that and it works great, just as your tutorial shows, however when I try to validate the page I get the above errors. The errors have come within the form generated by Dreamweaver 6. Since, I have validated the page online (w3c validator)... but still have these errors within Dreamweaver's validation.
