Fade for size jitter in shape dynamics doesn't work

I have CS6. 

 

I used to be able to fade out brushes under the Size Jitter in Shape Dynamics but can't now.  I open up CS3 and I can do it in there.  Any idea why it isn't working in CS6?

    Do you have the Minimum Diameter set to something other than 0%?

     

    Are you using a tablet or mouse?

     

    A screenshot of the brush panel might provide some more clues.

    Minimum Diameter is at 0, it has been before when fade worked. 

     

    Here is a screen shot.

    I have Fade at 50%, yet it hasn't changed.  I moved the slider for Mininum Diameter and nothing changed.

    Switch off the 'always use pressure for size' control in the Options bar.
