CGI form error

this is my form code embeded in a web page, i am using godady.com's cgi script,  am only getting entries f0r the the first feild and the name of the secod feild, no other information is being dellivered in the email, even the feild names are not being delivered...please help

    I think the problem might be related to using two words to name your fields.  The names you assign are essentially variables, and variable names cannot have spaces in them.  Try getting rid of the spaces in the id and name parameters.  Also, I would not use dashes in them either (Phone Number-2  change to PhoneNumber2, etc).
