Insert Image gets Placeholder Image

Whenever I insert an image (either with the menu or the Insert bar icon), I get the placeholder image instead.

 

I should be getting a file browser dialog, but I only get a 32x32 gray box. Maybe there is a preference somewhere that has changed?

 

I am using Dreamweaver CS 5.5, on a Mac with os 10.8.2

    Are you clicking on 'Insert > Image Objects > Image Placeholder'? or 'Insert > Image'?

     

    Image placeholder will, by default, give you a 32px square placeholder.

    Are you working within a Defined Site? 

     

    Have you experienced any other weird problems in DW?

    Start with #2, #4, #6, #10, #12.  If no joy, go to #14.
