    Is it no longer included?

Correct. Freeform was only part of AE for CS4 - CS5.5. These days you need to buy it.

If you upgraded from CS5/5.5 and you still have it installed, you can simply copy the effect inside your CS6 plugin folder. Not sure if this is a violation of Adobes EULA, so you might wait for Todds post.

Not sure what you are saying. The normal Freeform is merely CPU driven and doesn't require any specific graphics card. Only FreeformPro uses GPU acceleration, but since it uses OpenGL, not CUDA, it will run just fine even on ATI/ AMD cards...

FYI Freeform runs just fine on a Mac Mini, Mac Book (not pro) and a MBProR (full meal deal) in CS6. No problems at all with or without GPU acceleration.
