How do I add a pop up box?

I've created a form that customers can click on from our website to apply for an account.

 

We're having a few people struggle with the concept that they have to save the form and email it back to us (it seems like a simple concept, but apparently not.)

 

I want to create a pop up box that appears after they first click through to the PDF that says something like "Please fill this form out, save it to your desktop and email it back to example@example.com" that they have to click 'Okay' on to get to the form underneath.

 

How should I go about doing this?

    In Acrobat, go to Tools - JavaScript - Document JavaScripts. Create a new script called "init", and then remove the default code that shows up in the window that opens, and paste this code instead:

     

    app.alert("Please fill this form out, save it to your desktop and email it back to example@example.com",3);
