Form check boxes as group

Using Acrobat, how do I make check boxes that are dependent on each other. That is, let's say the form has an item with answers "Yes", "No" and "NA", and I add check boxes for each.  How do I set the boxes so that if the user checks one, the others will be deselected automatically?
