I'm developing a training program about various documents used in our company. I'm trying to insert Survey Questions as review activities after each document is reviewed. Here's the general flow of the program:

Survey question(s)/Review activity
Feedback screen - this is a screen I created since Survey questions don't provide feedback
Document field review
Survey question(s)/Review activity
Feedback screen
This repeats until all forms are reviewed
My challenge: The first document works perfectly. When the "Next" button is clicked after the second (third, fourth, etc) document is reviewed, the program skips the Survey Question(s) and goes straight to the activity's feedback screen. It's like it isn't recognizing the survey questions.
The next button is programmed to advance to the next slide (the first survey question). What am I doing wrong?
