Why can't get my form to work on the site?

Using Dreamweaver I downloaded a form with a php sheet to a company site. I copied the action from the current site pasted it into the action of my php and then saved it to my remote server. When I tested it I got a blank sheet. Then I pasted it in the form.html action box. I tested it and got error message " site not found" Can someone please help me?

    Hey thanks for the link to fix issues with Dreamweaver.  I was having the exact same problem as jess and I've been working with various websites trying to use this form.  My web design site and christmas toys product site will benefit greatly from your help. Thanks again!

    Which version of Contribute are you using?

    Can you explain the problem ? (like while connecting to website using Contribite... , OR while edit a page in Contribute / browser ... etc   elaborate in steps.)

    Thanks for the e-mai, but we solved the problem. Seems we had the sever number from yahoo.
