Urgent help required on Review mode of Captivate 5.5 Quiz

I am facing some issue with review mode of captivate quiz. After attempting the quiz and clicking the review button, I have noticed that, some questions failed to indicate selected option. Moreover when I reach to result screen in review mode the Score has also been changed.

Any suggestion or guidance is highly appreciated.
