Disable submit button when form is sent via email?

I have a pdf form that gets filled out and then submitted via email to my office. 

 

Once the form is received it is kept in electronic files.  I'd like to have the submit button automatically disable in the form that is received via email. 

 

The original form will stay the same so it can be completed and submitted again.  Does this make sense it's difficult to explain?
