I'm trying to figure out if the stamp tool is the best way to do the following:
I need a repeatable "stamp" that I can add to a document, then edit or add text to.
I need to fill in the blanks in several area, as well as X in one of several tick boxes.
I have been able to make a stamp with the correct info, but don't know how to add editable text and check boxes.
Any suggestions?

The usual way to do this is to use a custom JavaScript-based dialog to prompt the user for this information just prior to the stamp being placed. Setting up custom dialogs can be complicated, so there's is a learning curve involved. The best and most complete information is available at the subscription site http://www.pdfscripting.com.
