How do I submit a form with attachments to a ADEP process using WSDL

hey all,

I need to submit a PDF static form and invoke a LC/ADEP process.  Can someone mock something up to show me, I have read everything I can find, and nothing shows how to create a button that submits the entire PDF over WSDL and invokes a process.  NEED HELP!!!!!!!
