Are you using FormsCentral (web form) or a PDF form created using Acrobat?

    Thanks for the detail.

Later this week we will providing the ability to create a fillable PDF form from the form you created in FormsCentral. I think this will address your use case. That will be a paid feature of the service.

Do you want the data collected within FormsCentral? Or have it all managed in a file by you?

Currently there isn't a way for you to pre-fill the web form before the designer fills it out. With a PDF form this workflow would be possible but you'll have to wait a couple of days for it.

We just added the ability to distribute your form as a fillable PDF form and have all of the data submitted back to FormsCentral. I wrote up a forum post that explains it in more detail: http://forums.adobe.com/thread/994199

Give it a try and please send us your feedback on it.
