Sounds like it might be browser related. What I would do is:

test between IE and Chrome
test on different versions of each
check security restrictions on the machines where it did not work. If it is a security restriction then find out what you can change in your software to accommodate those restrictions or alternatively place a caveat at the start regarding requirements for running your training. (i.e browser requirements, security levels, internet security software restrictions etc...)

I don't know if this will fix your problem but it sounded to me it was probably something like this.

Quiz settings have been set to infinite attempts for a failing quiz and the checkbox is clicked to display the "Retake Quiz" button on the slide.

That said, I'm getting reports that some users are not seeing the Retake Quiz button when they fail, so they are unable to retake it. This seems to be an intermittent issue, and I haven't been able to replicate it on my end testing with various internet browsers. I also ran it through SCORM Cloud and it didn't report any issues. I'm using SCORM 1.2, and published as SWF.

Does anyone know what I'm doing wrong? Thanks!
