With a form created in Acrobat, it's only possible with Reader if using version 11, and the image has to first be converted to PDF, which is fairly simple to do nowadays. An alternative is to use an XFA form created in LiveCycle Designer, which can include an image field that supports the JPEG, PNG, TIFF, and GIF formats.

I am creating a form and I want a blank spot that the User of the Form can Insert a Picture.

I know I can insert a picture on the form, but I want the user to be able to do that.
