Adobe Flash player add-on continually re-appears

Webpages want to run the following add-on continually: 'Adobe Flash player' from 'Adobe Systems Incorporated'

When I click 'all websites', the download appears to run, then the same pop-up immediately re-appears.

This happens all the time in IE-11, occasionally in Foxfire.
