Leaving PS and coming back in causes the tools to not function. Heal and brush do nothing. Latest OSX and latest PS. Also exporting from LR into PS.

Could you please share your system info here by going to Help > System Info?
Also, please try resetting the preferences of Photoshop & let us know if it helps: Preferences in Photoshop
