Can't delete window - EVER!

PS CS6 on a Mac running High Sierra. Though this has been going on a while.

I delete this particular window that was a jpg. Then close PS. EVERY time I reopen PS that window reappears. I even tried removing the contents and saving, then deleting. Now it just reappears as an empty window. I've tolerated it for 18 months. Now I'm wondering if the cure is less painful than that file poking me in the eye all the time. Irony of it all? It was a political photoshopping of a despicable clown. I guess they got the last word on me. LOL.

I can later when I am home, but the window is simply a blank window (because I saved it that way). It has the checkerboard background as is set up in my preferences.

Incidentally, I am not prompted to save changes when I shut down PS.
